The Trustees are pleased to provide you with this Comprehensive Benefits Agreement which describes your benefits provided through the auspices of the Port Chester Teachers Association Welfare Trust Fund.
Included in this agreement are all the trust fund benefits - dental, vision, legal, financial advisory, and life insurance in one agreement. This agreement contains details of these benefits including enrollment, eligibility, submitting claims, and other information concerning Trust Fund procedures.
To the extent that this agreement describes an insured benefit (e.g., life insurance), the group insurance contract specifies the exact benefit provided, and the language of the insurance contract will govern in the event of any inconsistency between it and the language of this agreement. In addition, state approved certificates from First Reliance Standard Life Insurance Company are available upon request.
We suggest that you read this agreement carefully and keep it available so that you can refer to it in the future.
